Asthma Care Navigator

BRIEF SUMMARY: The Asthma Care Navigator demonstrates critical thinking to assess and meet the needs of patients by providing care coordination throughout the continuum of care. As the main point of contact for patients and providers across care settings, the focus is to manage patients in the ambulatory setting and reduce acute care & ED use. The Asthma Care Navigator may participate in department and hospital committees, patient care evaluation, performance and quality improvement, and educational programs. Additionally, in this role, promotion of disease management and preventative care for patients with chronic disease is a priority.
Demonstrated knowledge and skills necessary to provide care appropriate to the age of the patient served on his or her assigned department/unit. The individual must demonstrate knowledge of the physiological, psychosocial, and emotional needs of the patient as related to age, possess the ability to assess data reflective of the patient's status, interpret the appropriate information needed to identify each patient's requirements, and to provide the care needed as described in PMHD's policies and procedures.
RequirementsESSENTIAL FUNCTIONS:
1. Perform clinical/technical/service/administrative tasks:
2. Assists patients to navigate and utilize the healthcare system appropriately to encourage increased self-management of their care including utilizing appropriate available resources.
3. Identifies potential and realized barriers to care (e.g. transportation, low literacy, financial, insurance, etc.) and facilitates referrals as appropriate to mitigate barriers.
4. Facilitates timely scheduling of Outpatient appointments with the primary care physician and/or other Specialty, diagnostic testing, and procedures to expedite the plan of care and promote continuity of care.
5. Educate patients and/or families as to the nature of disease and to provide instruction on proper care and treatment.
6. Develops and coordinates a model of access and care benefiting patients, providers and family members.
7. Develops, identifies, tracks and conducts outreach to patients who would benefit from a coordination of care approach based on established patient identifiers. Identifies high risk patients and manages them with the goal of improving quality and reducing costs.
8. Establishes relationships with patients/families to set reasonable expectations for care as evidenced by reduced Emergency Department use, unnecessary hospitalizations and increased use of outpatient resources.
9. Collaborate with medical providers, patient care staff and clinic management in the planning and implementation of patient and staff education.
10. Design or participate in the creation of health education materials.
11. Ensures prompt documentation of patient encounters and services provided.
12. Performs other duties as assigned or requested.
EDUCATION, KNOWLEDGE, ABILITIES, EXPERIENCE AND SKILLS:
1. Graduate of an AMA approved school of Respiratory Therapy unless licensed under the grandfather clause of the California Respiratory Care Act of 1982.
LICENSES AND CERTIFICATIONS:
1. Current California Respiratory Care Practitioner license required.
2. Certified Asthma Educator by National Asthma Educator Certification Board. Holds a current certificate or obtains within 3 months of hire.
3. Current American Heart Association Basic Life Support certification required.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
