Reports to Project Coordinator Develops one on one asthma and air quality health presentations to program participants Conducts Home Visits Interviews clients Collects data pertinent to project activities Makes referrals to community resources Compiles reports Conducts phone interviews with program participants Distributes health information to program participants Attends team meetings Attends staff development training Acts as a resource for program participants.
Is able to interpret Asthma Action Plans Must provide Spanish language services as needed 90 day probation period at 30 hrs/week.

Ensures confidentiality and protection of privacy Follows personal safety protocols at all times Submits reports in a timely fashion Collects data accurately Conveys effective communication and courtesy at all times Actively participates in on-going project activities Respects patient-provider privilege Observes ethical standards at all time Able to communicate effectively, orally and in writing Excellent organizational skills Ability to exercise good judgment Ability to manage multiple tasks Ability to work independently and as a contributing member of a team Ability to relate to persons of diverse backgrounds and functioning levels Work Hours:
Full-time, Not to exceed 8 hours per day or 40 hours per week, Mon-Fri.
Occasional evenings and weekends, the schedule may vary between outreach and other activities.
Requirements:
Ability to communicate effectively with community members and other partners.
Strong interpersonal skills to meet with the community to establish trust and confidence and become familiar with its character, needs, leaders and their interests.
Bilingual; Spanish Proficient Familiarity with Google Apps (Mail, Docs, Sheets, Drive) Proficient in Microsoft Office (preferred) California Driver's License Personal Vehicle with Valid Insurance Certificate Bachelor's degree or equivalent professional experience One (1) year experience working with community/family health Knowledge of Asthma-related problems is helpful Knowledge of available community resources a plus Occasional travel required Knowledge of Air Quality Issues Job Type:
